Create a collaborative postmortem
Prepare evidence, start a shared document, and submit a version for review.

Before you start
Intended audience
Incident owners and responders who write a postmortem together.
Required permissions
You need permission to create and edit the incident postmortem.
Prerequisites
- Open a resolved incident.
- Make sure that its key evidence is recorded.
Procedure
Prepare the draft

Open Postmortem
Open the incident. Select Postmortem.

Review included evidence
Review Included evidence. All available evidence is selected by default.

Add operator context
Use Operator context for decisions, uncertainty, or constraints missing from the incident record.
Step 4 of 7
Choose how to begin
Select Write from scratch for direct editing. Select Prepare assistant proposal to capture evidence and request a proposed revision.
Collaborate and submit
Step 5 of 7
Review the assistant proposal
Copy revision instructions into your connected local assistant. Copying does not start the assistant. Review the returned proposal, then select Apply to working draft.

Edit together
Select Edit document. Use the shared editor and its document controls. Wait until Tellagen shows Saved draft before you submit. This confirms that the backend saved the draft.
Step 7 of 7
Submit a version
Select Submit changes. Tellagen creates an immutable submitted version. Applying an assistant proposal only saves the working draft.
You are done when…
The incident has a synchronized postmortem document and a submitted version with reviewed evidence.
Troubleshooting
The postmortem is not ready to submit.
Add document content, wait for Saved draft, and confirm your edit permission. Section checks are advisory; check the claims against the evidence.
A collaborator cannot edit.
Confirm the user has access to the workspace and the room token was issued for the exact incident postmortem.
The evidence list contains an unsupported item.
Clear that item before preparing a proposal. Record better evidence when needed.
Changes are not synchronized.
Keep the editor open. The local draft remains in the editor while it reconnects. Submit only after Tellagen shows Saved draft. If that state does not return, ask a workspace manager to check collaboration availability.
